| Brief summary of what participants will be asked to do: | Nasal mucus (“nasal swab”) collected at enrollment: Non‑invasive swabbing of the anterior nares performed in a private area; takes approximately 1 minute to confirm positive COVID or Influenza.
Optional two additional nasal mucus specimens: Also collected non‑invasively; each collection takes ~1 minute.
Optional at‑home nasal swab collection (if COVID or Influenza positive): Participants may collect daily nasal mucus samples for up to 10 days using a provided kit. Each swab takes ~1 minute, and completed swabs are returned at the 1‑week blood draw visit at CTSU.
Optional 30‑minute breath collection performed at the first encounter. Participants breathe normally into a breath collection device positioned approximately 5 cm from the face; the procedure takes about 30 minutes and is conducted in a private area or designated space.
Participants with confirmed COVID‑19 or Influenza may elect to provide up to five blood samples (~10 mL each) on the following schedule:
At the time of presentation at the healthcare encounter
Approximately 1 week after diagnosis
Approximately 1 month after diagnosis
Approximately 3 months after diagnosis
Approximately 6 months after diagnosis
Baseline survey: Collects demographics, vaccination history, brief medical history, household composition, and current influenza‑like illness in the household. Takes ~10 minutes.
Follow‑up survey at each blood draw: Collects updated information, including new vaccinations, medical encounters, and any changes since the baseline survey. Takes ~5 minutes per follow‑up.
W‑9 Form: Completed by all participants to enable research incentive payment (research‑only requirement).
REDCap Electronic Consent Form: All participants sign the study consent electronically; a copy is emailed to participants.
